redirect with data included after signing

i have remote computer service business 3 websites.

what need this: when customer on 1 of sites chooses service plan, either directed page on our site support agreement or sent link site support agreement or emailed similarly. once sign agreement, contains name of site on, plan chosen, price of it, , number of computers supported plan, other usual info... must sent payment page on main site. page should receive 4 fields in redirect can grab data $_get or $_post or $_request in php , populate payment form.

docusign claims have feature , seemed relatively easy set up; however, after 4 hours , 3 support calls yesterday, not work.

if can , it's easy me use, business.

fyi, site name , plan name fields on support agreement need drop-downs ideally.
