Checkbox signed instead of checked

i used checkbox in pdf. prepared pdf signature using acrobat pro dc.

sent client , instead of check in box, came signature... wtf?

i see mistake made, think:

the first check box signature box?? when click 'preview' works check box.

when send out... got signature back. super frustrating!

how did make mistake, can never make again!?!

right-click properties on box doesn't seem help...
